Registering a Signal Booster with Telus

I have limited cell coverage at my house.  I purchased a SureCall Signal Booster (https://www.surecall.com/CA/signal-booster/home-office/FLARE-3/) and did a quick test to make sure that it is working.

According to the SureCall documentation, it says I have to register the device with my provider.  Does anyone know how I would do this with Telus?

The registration will be for the US. In the states the FCC requires signal boosters to be registered with the carriers, in Canada you’re fine.

A signal booster should not require registration with Telus. It is simply a radio transceiver set.

There is only one approved "booster" for use on the TELUS network (Cel-Fi Go X), Surecall is not.

There is a registration required:

Please answer the questions on this form PRIOR to booster installation. The TELUS Team will gauge available capacity and interference on existing sites to approve / deny the request. Note: According to ISED Article CPC-2-1-05 on Zone Enhancers, Section 6.1 states that: "Purchasers of these devices
