Need help retrieving a puk for my sim

Two weeks ago telus gave me a new puk as I had problems prior same issues but was able to make phone calls
This time I can not make or recieve calls
Not a happy person right now
This is my life line as I live on a small Island and donot have access to a telus store
Shows it is enabled but my phone says differently
The puk worked for one day only
I tried taking it out and putting back and tried the puk again and did not work
I only have one chance left before it locks permanently.
Been unable to get ahold of you people for 2 weeks.
