At the top of the page, choose Support, Mobility, then Device Activation, Changes and Upgrades, and there are a number of descriptions of the unlock process.
Basically, the phone must be or recently have been on the Telus network, and the Telus customer /owner of the phone must request the unlock and pay the fees.
