Many of us have been plagued by this code...
The PUK code for your device is provided by your carrier (Telus, in this case). You need to call the Telus support line and ask them for the code. Make sure you write it down and store it somewhere safe so you'll have it handy in case it happens again.

There's a different PUK code for each sim card. Only your carrier can give you the code. There is no cost for asking your PUK code, but they need to validate that you are the account administrator.
