Fees & notice to cancel phone with month to month (no contract) billing?

I am no longer on contract and just paying month to month now but I would like to cancel my phone, these are my questions: 

1. I have read there is a $50 cancellation fee for early termination but since my contract is up I wouldn't be cancelling early, does this still apply?

2. Would I just pay the prorated fee of what I have used so far in my billing cycle up to today's date and that is all or....?

3. Do I also need to give them 30 days notice since I am no longer on a contract?
