Switch Disney+ account linked with my stream+ sub

Hello there,

I subscribed to Stream+ today and was trying to activate Disney+. I accidently activated it on an older account and would like to disconnect Stream+ from this old account and activate on a different account. 

Is it possible to switch Disney+ account after activation? if so, how?
