Whenever I try to create an account on optik TV it says "Failed creating account". How do I fix this?

Can you provide a bit more detail?
Is this the Optic Go App, or on the TV itself?

If you can't create an Optik Account through the TV interface using either a new, or previously chosen login and password, you could try to delete and reset the account. If that too fails, then I'd suggest calling in to see if it can be sorted out from Telus' end of things.
I had no trouble setting it up a years ago; just make sure you are using the same login and password throughout, so you can connect your other devices to the account.
