Have you reviewed your invoice through 'My Account'? It will have the details of your charges in fine detail. You'll need to connect with Telus directly to get your phone unlocked.  You can call as described in the 'Contact Us' or reach them through their Facebook page or Twitter account. The latter two options may be the most time efficient.
