Spontaneous paper bill?

For some reason Telus spontaneously sent me a paper bill last month for no apparent reason, and charged me $3 for it, even though I've always been on e.bill. I still got the normal email bill notice, and my account billing method setting hadn't changed. I used Chat to get the charge fixed without problems, but I wonder if anyone else had that happen? Could it be some glitch of switching to a new system?
