Modem/Router intermittently hanging

I have had this intermittent problem (once or twice a day or even up to 3 weeks between failures) since signing up to Telus Internet and Optic TV. I've had Telus replace the T2200H modem with no improvement. 

 

When the LAN fails, I get lots of these error messages in the T2200H log: "eth1: received packet with  own address as source address".

 

The problem can be cleared by disconnecting my ethernet switch from the T2200H and everything comes clear.

 

Has anyone else had the same kind of problem? Any thoughts would be appreciated.

Might you have a bad switch?  

It might be worth trying a fresh one to see if that is the problem.

I don't think so. I've replaced the switch and patch cables.
