What does my modem look like?
Is it the thing they installed in my front hall closet or the little black box next to tv?

Likely the box in the closet. The one next to the TV is most likely your Optik PVR.

That statement is a generic diagnosis and not a description of the problem. It would be easier for others to help you if you provide a description of the specific symptoms you are experiencing. Which devices you are using, how are they connected, what you are doing at the time, etc.
