Upgrade equipment

Trying to update to the new box with this link, but it’s not working. Asks for my address but no where to enter it. https://www.telus.com/en/tv/optik/tvupgrade

Were you asked to log into your Telus account, I was just now? Address, as in street address?

When I go to the page there is a popup that says to enter my home address but there is no field to enter it.
