PureFibre installation request for condo building?

Your condo board has to reach out to Telus to get it run in to the building. Typically it is done at no cost to the building. My building in Edmonton had it installed a few years back.

There used to be a form the board could fill out on the website but I don't see anything on the above page. TELUS_Support​ , has the process changed or has the link to the form the board rep has to fill out been moved?
